COVID-19 Wellness Form

Due to the COVID-19 pandemic, please know that all public surfaces at Hang On To The Dream have been cleaned and sanitized to the best of our ability.

If you are concerned about catching COVID-19, or if you have a pre-existing medical condition that puts you at greater risk of catching COVID-19, you are advised not to enter the premises. 

 

Further, if you have come in contact with anyone who has tested positive for COVID-19, have personally experienced any symptoms of COVID-19 (such as a fever, cough, or shortness of breath), or have been in contact with anyone who has experienced the above symptoms and is awaiting testing for COVID-19, you are not permitted to enter the office or studio areas. You are more than welcome to contact us again in the future when your condition and/or circumstances change.    

NOTE:

 

All persons filming or recording at the Hang On To The Dream studio MUST complete this form within a 24 hour period prior to entering the studio.


At this time, no more than 4 (four) people will be allowed in the production studio at any given time, and no more than 10 (ten) people will be allowed in the Hang On To The Dream office at any given time.

 

​Everyone present MUST maintain a required social distance of no less than 6 feet while on the property, unless filming or recording, and ONLY with the mutual agreement of all parties involved.
 

Additionally, you MUST wear a face mask/covering while in the studio and office areas, unless filming/recording.



All Hang On To The Dream clients and guests are required to submit answers to the questions below prior to entering the studio.  Access to the studio is subject to approval.

In the 14 days preceding the date of your filming/recording, have you been in contact with anyone who has experienced the above symptoms and is awaiting testing for COVID-19? 
In the 14 days preceding the date of your filming/recording, have you come into contact with anyone who has tested positive for COVID-19?
In the 14 days preceding the date of your filming/recording, have you personally experienced any symptoms of COVID-19 such as a fever, cough, or shortness of breath?
